The answer is this. Fred and Sue have just done an English test. Sue wrote "he had had had breakfast.", which is incorrect, and Fred wrote "He had had breakfast.", which is possible in the past perfect.

 

In this context, the following sentence is possible:

 

Fred, where Sue had "had had had", had had "had had". "Had had" had got to be the correct answer.
